How to make a referral

We understand that entrusting the care of a loved one with profound disabilities is an emotional and deeply personal decision. At Martha Trust, we’re here to make that journey as reassuring and empowering as possible—offering a safe, nurturing environment where individuals can flourish, build friendships, and live life to the fullest.

We provide care for young people and adults aged 18 and over who are unable to walk unaided and have moderate to profound learning and physical disabilities. Referrals are welcomed from family members, care managers, community nurses, GPs and other professionals.

Our admissions process is straightforward, with friendly guidance at every step. Here’s how it works once we received your initial enquiry:

  • We’ll reach out to invite you for a personal visit to one of our homes, offering you a chance to experience life at Martha first-hand. It’s the perfect opportunity to see whether our environment feels right for your loved one.
  • Our Home Manager will visit the applicant in their current setting—whether it’s a family home, care service, or educational placement—to get a sense of their needs and preferences.
  • We’ll carry out a comprehensive Individual Needs Assessment, covering both nursing and social care. The family’s insight is invaluable here, and your input will help shape this step.
  • Our Home Manager and Clinical Associate Director carefully review the application to decide if we are a suitable placement for your relative.
  • If it’s the right match, our Funding Manager will begin discussions with the relevant funding authority to secure financial support.
  • Once funding is agreed, we’ll move forward with a carefully planned transition.
  • A series of introductory visits will be arranged—starting small and gradually building up to a longer stay, helping your loved one ease into their new surroundings.
  • We’ll work together to create a bespoke Life Plan, tailored to individual preferences and support needs, including consent forms, movement and handling requirements, and more.
  • Finally, we’ll agree on a move-in date and encourage close family involvement throughout the early stages of settling in.
